  • ARC/Modification Request Form (PDF)» 
    Log in to the online portal to submit an ARC (Architectural Request For Change) request. 
    Use this form to request approval from the board for any modifications that affect the exterior of your home. Requests are reviewed at the monthly board meetings. If your modification is an urgent issue (e.g.: broken door or window), please note this on the form.

    Please be sure there is a completed form on file for your unit, with all of your current contact information in case of emergency.
  • Tenant Information Form (DOC) »
    Co-owners who rent out their units must provide contact information for their tenants and acknowledge that the tenants have been given Association documents and provide a copy of their lease.
  • ACH Payment Form (PDF) »
    Complete this form and mail to Herriman & Associates to set up direct bank payments.
  • Designated Voter Form (PDF) »
    Each unit should have one designated voter to participate in voting on selection of board members, bylaws, etc. Complete this form and return to Herriman.
  • Proxy Form (DOC)  or Proxy Form (PDF) »
    To meet our quorum and hold a valid meeting, we need 35% of our co-owners to attend or send a signed proxy, so your participation is critical. Please sign and give your form to someone who is attending or to any designate a board member. (Please note: in order for the proxy to be valid, you need to have also turned in a Designated Voter Form.)
  • Owner-Tended Garden Policy and Application Form (PDF) »
    As a condo association, all the common limited common areas including the garden beds near each unit are the responsibility of the association.  Recently the board wrote a new policy allowing co-owners to request permission from the board to maintain their own front garden beds, in agreement with the provided guildelines. Policies

The Board periodically reviews and updates our written policies. The following have been approved and are in effect:
